NDIS Community Nursing in Ryde & Northern Sydney

NDIS community nursing refers to clinical nursing support delivered in a participant’s home or community setting. These services are typically provided by Registered Nurses and may include wound care, medication administration, health monitoring and other clinical supports required to help participants manage health needs related to their disability.

Yes. The NDIS may fund nursing services when they are considered reasonable and necessary to support a participant’s disability-related health needs. Funding availability depends on the participant’s NDIS plan and individual support needs.

Our Registered Nurses provide a range of NDIS community nursing services, including: • wound care and dressing changes • medication administration and monitoring • chronic condition monitoring • nursing health assessments • complex nursing care where appropriate All clinical services are delivered by Registered Nurses.

Yes. Our nursing services are delivered through home visits, allowing participants to receive professional clinical care within the comfort of their own home.

Our community nursing services currently focus on the Ryde area of Northern Sydney, including: • Ryde • Gladesville • Hunters Hill • Putney • West Ryde • North Ryde Additional nearby areas may be considered depending on availability.

Yes. Our Registered Nurses can support participants who require advanced or complex nursing care, depending on the individual’s clinical needs. Services may include ongoing monitoring and nursing support for participants living with complex medical conditions.

Yes. Support Coordinators, healthcare professionals, participants and families are all welcome to contact us regarding nursing support. More information about referrals is available on our Referral Information part.

A formal referral is not always required for initial enquiries. Participants, families or healthcare professionals can contact us directly to discuss nursing support needs.

Yes. Community nursing services can support participants following hospital discharge, helping ensure safe recovery and ongoing monitoring at home.

In addition to NDIS services, we may also provide private nursing support where certain services are not covered under NDIS funding. Please contact us to discuss private service options.
