NDIS Community Nursing
Registered Nurse in Ryde & Northern Sydney
Blossoming Horizon provides a wide range of therapy services tailored to meet the needs of children and families.
Wound Care & Dressing Management
Professional wound care delivered by Registered Nurses. Services include:
• Post-surgical wound care
• Chronic wound management
• Pressure injury management
• Dressing changes
• Wound monitoring and infection prevention
Medication Administration & Monitoring
Safe medication support provided in accordance with clinical standards. Services may include:
• Medication administration
• Injection support
• Medication monitoring
• Clinical medication oversight
Chronic Condition Monitoring
Support for participants living with long-term health conditions. Examples include:
• Diabetes monitoring
• Blood pressure monitoring
• Chronic disease monitoring
• Ongoing health observation
Complex Nursing Care
Some participants require advanced nursing support due to complex health needs or disability-related medical conditions. Our Registered Nurses may assist with:
• complex respiratory monitoring
• enteral feeding support
• catheter care
• advanced medication administration
• ongoing clinical monitoring
These services are provided by Registered Nurses with appropriate training and clinical experience.
Participant-Centred Care
Our nursing services focus on providing safe, practical and personalised care that supports each participant’s goals and daily living needs. We work collaboratively with:
• families and carers
• allied health professionals
• primary healthcare providers
to ensure care plans remain safe, responsive and clinically appropriate.

Our community nursing services currently focus on the Ryde area of Northern Sydney, including:

• Ryde
• Hunters Hill

• West Ryde

This focused service area allows us to provide reliable scheduling and consistent nursing visits. Additional nearby areas may be considered depending on availability.
Private Nursing Services
In addition to NDIS services, we may also accommodate private nursing support when certain services are not covered under NDIS funding. Examples may include:
• additional wound care visits
• short-term post-hospital nursing support
• family-requested nursing visits
