For Support Coordinators and Healthcare Professionals
Our services support participants who require ongoing nursing care, wound management,
chronic condition monitoring, or complex health support at home.
Nursing Services Available
Our Registered Nurses provide a range of NDIS community nursing services, including:
• wound care and dressing management
• medication administration and monitoring
• chronic condition monitoring
• clinical health assessments
• complex nursing care where appropriate
All clinical services are delivered by Registered Nurses.

Service Area
Our nursing services currently focus on the Ryde area of Northern Sydney, including:
• Gladesville
• Ryde
• Hunters Hill
• Putney
• West Ryde
• North Ryde
Additional nearby areas may be considered depending on availability.

When to Refer
Support Coordinators may consider referral when a participant requires:
• regular wound care or dressing changes
• ongoing clinical monitoring
• medication administration support
• nursing support following hospital discharge
• complex nursing support related to disability or chronic health conditions

Information Helpful for Referral
To assist us in assessing service availability, the following information is helpful:
• participant suburb or service location
• brief description of nursing needs
• preferred visit frequency (if known)
• relevant clinical information (if available)
A formal referral form is not required for initial enquiries.

We value collaborative relationships with Support Coordinators and allied health professionals. Our goal is to provide reliable, clinically safe and responsive nursing support that aligns with the participant’s NDIS plan and care needs.
